RISE Project: Boston Program Saving Lives of People Released from Jail

Data from Massachusetts shows that people released from incarceration are 125 times more likely to die of overdose than the general population. Most of these deaths occur in the first few weeks after release. Boston Health Care for the Homeless Program created RISE to change that.

Breaking the Cycle

The RISE Project connects with incarcerated individuals who are living with opioid use disorder months before their release date. Working with people in Suffolk County jails, patients begin receiving case management and support groups while still detained.

Seamless Care

When they walk out of jail, care continues without interruption. This includes medication for addiction treatment, Hepatitis C treatment, help obtaining identification, mental health assessment, legal case management, and housing applications.

A Different Way

Too often, incarceration, homelessness, opioid use, and overdose form a vicious cycle. RISE breaks that cycle by treating addiction as a health condition that deserves care and support.
